The results from this questionnaire will be used to improve the rate of infections at Healthy Hospital. With the aid of your participation, it would become much clear for me to take appropriate measures to reduce infections at Healthy Hospital. Survey Distribution Method There are a number of different methods to distribute and collect surveys but the method causing the least inconvenience to the staff supervisors must be chosen. Filing out surveys is not part of the job description of staff supervisors so their cooperation is essential to taking this research forward. Moreover staff supervisors may often have their hands full with problems of one kind or the other so the survey method should be convenient and non intrusive (Survey Bounty, 2012).

Possibly the best method to deliver such a survey would be on the internet with the survey being introduced by email. The email could serve similar to the cover letter and could contain the link to the actual online survey. All the staff supervisor would need to do is to click on the link to go to the survey online. Each email would be sent to a unique supervisor so the link in the email could be configured to hide the identities or to reveal the identities of each individual supervisor. Moreover as all staff supervisors are connected to the internet so it would be easy for them to simply click a link and get a survey done.

A number of free and paid services are available online to create surveys such as Survey Monkey, Free Online Surveys, Kwik Surveys etc. Another large advantage of using an online survey is
