Final Organizational QI Plan Lecturer Executive Summary The Quality improvement plan of the Community Care, a hospital in the of Kawartha Lakes, demonstrates the facility’s determination to improve the quality of care services offered to the clients. The QIP enables a coordinated, systematic and a continuous approach to promoting the performance of the hospital through focusing on the mechanisms and processes addressing the hospital’s values. QIP will also outline the strategies, goals and mechanisms of ensuring optimal delivery of care, safety of clients and attaining satisfaction level of the clients. Eventually, the QIP will emphasize on the efforts on the support processes of care delivery to clients that will promote optimal outcomes to clients and effective practices of business. Within the hospital, every program and department is responsible for ensuring the activities of the QIP are implemented. The initiatives of QIP will be conducted as the activity of the organization’s Quality Management Committee. In the process of implementing this QIP, there will be regular assessment and monitoring, annual evaluations and quarterly reporting of the QIP as a journey of the QIP for the Community Care Center. The implementation process within the hospital may include development of the currently existing goals and addition of new objectives and goals that are based on emerging issues and priorities involving the Community Care Center. Introduction and purpose The Community Care Center of Kawartha Lakes in the County of Victoria is multi faceted and integrated organization that provides a wide range of community support programs and health services designed to support independence and promote health for all ages and life quality for both special needs and senior clients. The mission of the facility is to be the leader in the delivery of responsive, integrated health and support services for the community as well as collaborative designs that respect the individual independence and dignity. The facility strives to build a healthy, safe environment where staff, physicians and volunteers join hands in working to provide clients with quality services and care. Community Care Center provides services such as: health services for the community like programs of wellness, health promotion, and primary health care; support services to the community like meal programs, transportation, adult day programs, and in home maintenance and support; services of hospice like bereavement, palliative and grief support; and dental clinic. Goals and Objectives The Community Care Center defines quality as care and service provision that are effective, safe, accessible, client centered, equitable, efficient, and responsive. During service and care delivery, the organization observes compassion, respect, accountability and excellence. The organization has the following goals in order to meet its mission: To address all areas in which the services may possibly be improved To systematically and objectively evaluate and monitor the appropriateness and quality of the services that are carried out by the organization To focus on the goals that leans towards desirable outcomes To show attempts towards resolving the problems that are already identified To ensure sufficient efforts are made to deliver care that assure satisfaction and cooperation To assure accurate, complete, and appropriate documentation of the services and care of the process of delivery To ensure there is a compliance with the federal requirements and accrediting agencies regarding performance and assessment of quality The goals and objectives will be achieved through the accreditation process, review outcomes, initiatives, survey analysis, and performance appraisals of the committee of quality management. Achieving this will mean that all the employees of the organization participate in the quality improvement plan efforts. Scope, description and QI activities There are various programs, departments and activities that this QIP will impact on. Some of the QIP activities include: Accreditation Client, physician, staff, and volunteer satisfaction surveys All indirect and direct care services to the clients that affect the safety and health of the client Risk and safety management activities However, all the departments in the facility will be affected by the QIP since we are looking into improving the quality of the whole hospital. Some of the departments are the dental center, the community support service department, and the community health services department. All the programs of the Community Care Center will hence be in check with the QIP. Data collection tool In order to ensure quality within the Community Care Center, various data will be collected ranging from the satisfaction surveys of clients, physicians, staff and all the volunteer workers. The waiting time and consultation attention by the physicians to the clients will also be assessed. As mentioned before, our organization defines quality as services and care with is effective, safe, accessible, client centered, equitable, efficient, and responsive. Therefore these values will be measured in the services offered and the procedures from the point of entry to the point of exit. This will ensure the clients are served well by our staff, physicians and volunteers. Our support team as well as our staff and physicians will also be assessed to ensure they offer quality care services to our clients at good working conditions. Their level of work satisfaction will also be measured. QI Processes and Methodology The QIP process include: a quick fix process to be used in solving issues that do not require comprehensive approach and implementation of solutions; activities of quality assessment; forming quality improvement teams; creating a dashboard report; and using outside sources and databases available for our standards of professional practice. The method of implementing the QIP will include: Creating problems focused on evaluations and monitoring areas with suspected problems Assessment of the problem will be done with any best method that can best yield the results desired Prioritizing problems according to their impact on the services and care of the clients Having periodic evaluations of the corrective actions’ results Identification of problem analysis These processes and methods will be observed and monitored by the quality improvement committee. Comparative Databases, Benchmarks, and Professional Practice Standards The Community Care Center will use databases that are comparative to others and available for incorporation of the continuous assessment process which have a common practice and standard to the organization. The organization will use the following performance indicators to measure quality improvement: Timely availability of data Valid and reliable indicator Feasible indicator Actionable indicator The reflective of the priorities of our strategic planning The effect on high cost, high volume, and high risk Authority/Structure/Organization The QIP will be implemented by the Quality Improvement Committee who shall be responsible for coordinating the QIP, ensuring the completion of the review functions of the QIP, coordination of Accreditation Committee, prioritizing issues for review by the committee, ensuring availability of needed resources, analyzing data collected, appointing small teams to handle minor issues, reporting findings, recommendations and studies, and identifying needs of training and education to the staff. Other committee roles include: Communication: The Quality Management Team offers functions and oversight as a central house for improving data quality and the collected information within the organization. The committee is responsible for tracking aggregate data from other sources in order to remit the results and outcomes. Education: Every staff member has the authority and responsibility to participate in the QIP of the organization. The committee will provide education that pertains to QIP as needed. This will include QIP description and how to implement it. Annual Evaluation: The Quality Management Committee will be responsible for monitoring and evaluating the QIP annually in order to ensure effectiveness in the accomplishment of the goals and objectives.
