O Purpose of Letter: I am writing to refer (Patient’s for assessment of neck pain.Client Details:(Patient’s Name) is an active 69 year old and she denies previous episode of the neck condition. Onset of the neck pain is approximately two months ago. Treatment Summary:Although the patient has never responded effectively to the medication I gave her, there has been some little challenges in her neck because the pain still persists.

I have been treating the patient on a fortnight basis for the last two months mainly for neck pain but she still experiences some pain. I have used therapeutic modality on her neck also MRT, with mobilization and distraction. We further took an x-ray of her neck to make sure she does not have any pathological problem. We found that she has degenerative problem with decreased disc space especially from C3 to C7.Reason for referral:I have suspended further treatment until she undergoes a full medical assessment and therefore, I am referring the patient if we can manage her together .

Further evaluation from your end may be very helpful in improving her condition. I have attached the results of her neck X-ray and other medical results for your review.I really do appreciate your willingness to assist and kindly update me on her progress.
