Formal organizational arrangement 3 1 Structure The organization is organized in a hierarchical mannerwith a chain of command to be followed in reporting. The nursing department is headed by the nursing Vice president Camlle Hubbbbarg. The vice president is in charge of all the department’s activities and has the overall say as the manager of the department. The nursing department is further subdivided into two major departments; the pediatric department and the emergency department commonly referred to as an ER meaning emergency room. The ER is in charge of handling emergency cases in the institution while the pediatric department handles the inward pediatric cases as well as outpatient cases. The two departments are hierarchical with each being headed by a director. The two directors manage the affairs of the department with the help of two managers and two supervisors for each department. In general, the two departments are further subdivided into two so that below the director is the manager in charge of the line in the department and the supervisor is below the manager with the subordinates being at the bottom of the hierarchy. In the reporting system, the subordinates report to the supervisors, supervisors report to the line managers while the line managers report to their respective department heads. Both heads of departments are report to the nursing vice president. Just like any other hierarchical organization, MaiMed Center has order of flow of information. The vice president formulates the overall nursing department’s goals then the heads of the various heads of nursing department formulate ways of ensuring their respective departments coordinate to achieve the overall organizational goals. The managers are responsible for formulating the exact course of action to be undertaken by the department while the supervisors are responsible for ensuring implementation of the actual plan on the ground by the subordinates. Information in the organization is both vertical and horizontal. Deferent levels of management communicate with managers from their department and across to other departments. The organization’s management is able to communicate downwards to the subordinate while the subordinate are able to air their views through their supervisors up to the top management. Organizational structure of the department of nursing 3.1.2 Physical layout The organization is located in the outskirts of new York in a two storey building rented by the organization. The organization has a canteen next to the gate with a small shade and seats for employees to enjoy a snack. The ground floor of the building is specifically for the emergency cases while the first floor is for the pediatric department. The reception is located at the entrance of the building where any visitor can easily locate it and seek guidance on where to get assisted depending on their case. The hospital kitchen is located in the second floor with a dining room for the staff to have their meals when on duty. The hospital also has two theatres to handle the various cases it receives on a daily basis with one located on the ground floor and the second located in the first floor next to the wards. The staff also has a resting lounge where they can clean and freshen up before and after duty. The lounge consists of bathrooms and small cabinets for them to keep their private items when on duty. This demoralizes them due to the sharing of the lounge as there is no feeling of privacy especially when changing. Furthermore the situation of the kitchen on the first floor at times makes the patients in the wards uncomfortable due to the smell of food from the kitchen. The hospital has an ambulance that picks patients from their homes in case they are unable to get themselves to hospital and transfer others to other hospitals for referral when need be. Organizational schematic Ground floor First floor Entrance 3.1.3 Rules and policies One of the most visible hospital rule written on the wall of every department is the no smoking rule. Tobacco smoking is prohibited within the hospital premises due to the health effects of tobacco that may lead to the deterioration of the patients’ health. Other general rules are the visiting hours for admission cases. Patients require plenty of rest thus the time visitors are allowed to se then is limited to one hour in the morning between 9.00 am and 10.00 am and two hours in the evening between 4.00pm and 6.00 pm. The morning visit is slated just after the morning rounds checks to avoid delaying medications for the patients. Food from outside is also not allowed for patients admitted in the hospital apart from packed fresh milk and fruits. This is aimed at avoiding feeding patients with contaminated food thereby aggravating their medical condition. The hospital kitchen provides a variety of meals to suit the patients’ needs thus there is no need for home made food in the hospital. Use of protective gear is a must for both visitors and nurses when attending to patients to prevent spread of diseases from the patients to other people. International nursing guide practices are also followed by the organization. The nurses observe confidentiality in dealing with their patients. Patients’ information is handled with confidentiality and only their close relatives are allowed access to such information. The application of skill and professionalism is one of the requirements for the staff. Because of the beaurocracy and mistrust between the nurses and their seniors, nurses tend to follow rules just as a matter of duty. Thus there is no personal commitment on the part of the nurses. 3.1.4 Recruitment and selection The organization’s management under the leadership of the head nurse the vice president undertakes to recruit trained nurses from the field. The firm has a preference for fresh graduates from college to avoid paying high wages. Nurses who are already experienced demand higher pay and at times higher post. After carrying out interviews on potential candidates, the firm selects the best suited based on merit and personal expression of commitment and interest in the job. The academic grades ensure that the firm picks qualified nurses. Preference of fresh college graduates is however not the best way to recruit as they do not get the best experienced nurses. The lack of experience increases the trouble the organization goes through when recruiting new staff through the need for lengthy incorporation and learning the operation of an actual hospital. 3.1.5Reward system The organization pays salaries to its employees based on their level of management. The subordinates are the least paid employees in the organization. The reward system of the organization is not clear as there are no clear guidelines on the salaries of the employees and the addition of the salaries. The system is mainly corrupt and with a lot of anti-professional practices. For example, those having close relations with the senior VP are the ones who get a pay rise. This leaves most of the employees dissatisfied with the system but their effort to air their complaints is met with a deaf ear. Promotions are also effected on friendship grounds with the senior VP. This is a limiting factor to the organization as it gives the organization poor management through incompetent employees being promoted. This has led to administrative challenges in the organization. 3.1.7 Evaluation system There is no clear system of evaluation in the organization. The evaluations are done only once a year at the end of the year. This leaves a gap for improvement in the course of the year for lack of periodic evaluation. The annual evaluation is done based on the performance of duties in the course of the year, major achievements in handling patients special cases as well as punctuality in reporting to work. Absenteeism is also considered in the evaluation. This is aimed at informing the employees of their performance during the year and may also be used to as a basis for selecting employees to be laid off in time of financial trouble like now. 3.1.8 Training and development The organization generally employs qualified nurses thus the employees already have the basic skills required for work. For new employees, they undergo in house training on the operations of the organization. This is aimed at incorporating them into the organization. The organization also trains its employees periodically at a subsidized cost to help them be at the same pace with changes in the nursing practice. The firm also gives them a chance to study on part time basis for those wishing to further their studies. This is made possible through friendly arrangement of duties to enable those undertaking evening classes work during the day and study in the evening. Read More
