For this reason, different forms of written communications were formulated simply to be able to handle the needs of keeping the record clear for better communication procedures taken into consideration based on facility-to-facility connection that links the different hospital institutions involved in a certain medical case together so as to create a more progressive intervention process on the part of the healthcare professionals handling the situation of the patients. 1.2 background of the study In the field of healthcare personnel and institution responsibilities, communication plays a great role that affects the entire system’s operations. With the said involvement, healthcare officials giving care to a patient who is under a long-time schedule of medication needs to be well supported through the updates that are sent between the said institutions and even between personnel-to-personnel connection so as to be able to monitor the developments of the patient as they undergo the medication treatments that they are supposed to receive. 1.3 statement of the problem Usually, medical institutions that are involved in long-time cases of medical treatments are in need of being connected to each other for the sake of continuously being able to communicate together so as to have a background as to how a particular patient fares based on the record of medical development that an individual actually takes into mannered improvement in connection with the medical situation that they have to deal with. Hence, to be able to solve this problem, it is rather important to take into consideration that need to create an effective approach to finding the right process of communication smoothly processed for the sake of the health improvement of the patients being treated. 1.4 hypothesis It is believed by the researcher that through this particular study, it could be perceived that the operational use of the SBAR or Situation Background Assessment Recommendation, the process of assisting a patient for his or her faster recovery would become much of an easier task for the medical personnel involved in the said medical cases. 1.5 scope and limitation of the study Based from researches and observational procedures, this particular paper shall deal with the issues related to the problem in a way that it would be safe way from any ethical issues that may hinder it from gaining the right kind of resources needed to complete the entire piece. It could be noted though that through this, the issues of operational assessment on the part of the medical status of the patients being treated with the situation. Moreover, it could be observed that since the researchers are not allowed to intervene in the process through asking the patients themselves, this study might not be able to measure the competence level of the use of the SBAR communication tool based on the views of the patient. 1.6 significance of the study It is obviously true that this study is aimed in making a great intervention process as to how the SBAR communication tool could provide the best kind of assistance to the people involved in the medical cases being tried to be assessed through the said communication approach. It could be noted that through this study, proving the competence of the SBAR tool in completing the medical treatment needed to be received by a patient successfully could further be expected. Chapter 2 Review of Related Literature In the field of giving medical care to the people, it is rather important to consider how the patients are being satisfied with the type of services that they are receiving from their caregivers. Undoubtedly, through the years, caregivers such as nurses and other healthcare professionals try much to reach the goal of becoming highly involved in mastering the rightful process of servicing to their clients, the patients. What then are the necessary points that need consideration in the medical official’s search for the finest service that would satisfy both the needs and demands of the people? There are at least three major areas of focus that needs to be discussed when this kind of issue comes into light. The said elements of fine medical service are noted as follows: (A) Creation of fine policies that could serve as guidelines for the given service: This shall involve the different points of consideration that needs attention when one particular medical staff is giving health care assistance to a patient. Through the policies set to assist in the process, the standard procedure of servicing the clients should be observed strictly. (AHQR) (B) Creation of procedural approach for decision making in several special health cases: Special health cases usually involve emergency decisions. The process of making decisions is at times quite unpredictable, especially if the case to be treated is abrupt. Thus, a standard procedure should be set, especially when no one around is able to decide for an unconscious patient. (AHQR) (C) Involving patient and patient’s relatives in coming up with the necessary decisions about treatments: The patient’s relatives or close loved ones should be given the chance to help in the decision making pertaining to what particular manner of treatment would be accepted by the family as a process of health assistance applied on the patient. This is especially applicable with serious illnesses and other terminal medical cases. (AHQR) With the discussion presented, it could be observed that there are three major approaches in service that needs consideration on the part of the healthcare professionals when it comes to satisfying their clients. These three elements particularly involve fine decision-making on the part of the caregivers themselves and the involvement of the family within the process to give regard to their importance in the situation. Obviously, the procedure of extending the service to satisfy the client’s values as the major focus of the service involves the major action on the part of the administrator’s implementation of the necessary guidelines needed to assist the personnel in giving considerable type of service to their customers who are naturally the patients wanting the best satisfactory assistance and care that they need to survive the health issues that they are particularly facing. The SBAR system of communication was rather introduced to the medical society so as to assist them in coming up with a progressive process of handling the medical issues consistently towards having a well communicated connection between the institutions and personnel involved in the process of assisting the patients recover from their ailments. (Trentham, et al, 2008, 31) It is through this that the SBAR communication tool is expected to work well. Undeniably, the process of communicating among the people most involved in such procedures makes a great impact on how the medical personnel are able to better intervene in the process of recovery of the patients. (Karima, et al, 2008, 76) Through teamwork, SBAR communication tool utilization would certainly make it easier for the medical team to work on the process much easily as expected. Chapter 3 Methodology 3.1 research design Based on actual situations of using the SBAR communication tool, this research shall make use of actual experiences as basis of proof that links the entire study towards the particular claims that the research materials reviewed herein pertains to. Obviously, basing from the researches handled in the precious section, the operational success of any medical institution could be based on their capability of following through the rules through establishing teamwork in coming up with the most improved process of communication. The development of the SBAR communication tool naturally creates a better sense of teamwork among personnel as well as institutions working on a certain case together. Hence, to prove this particular claim, this research shall utilize both literary basis of facts and the actual experiences of the medical staff in handling issues such as long-time hospitalization and medication schedules among patients that they need to attend to. 3.2 respondents The respondents to the interview questioning procedure that the researcher shall handle herein shall involve medical authorities as well as medical personnel handling the actual cases of patient-to-caregiver relationship. TO instill the importance of the SBAR communication tool in the process of assessing the situation of the patients and giving them ample care from one handler to another is a sure notable process that could be carried on to ensure that the said communication tool is indeed competent enough to handle the information needs of the people involved in creating the most comprehensive procedures in healing or helping primary patients recover from their illnesses. 3.3 procedures To proceed with the process, a letter of notice shall be sent to the administration of the healthcare facilities to be targeted for staff interview procedures. The said letter shall state the mission of the researcher in handling this particular study and why the participation of the institution in the process is highly acknowledged by the researcher. Furthermore, the letter shall also consist of a permission request as well as recommendation form as to who among the healthcare institution’s staff does the administration think is well-fit to answer the questions that are to be asked by the researcher. From this phase, it is expected that as the administration permits the process to be handled, the researcher shall adhere to any rules or limitations that the administration shall set for the application of the process among the staffs that are to be interviewed. 3.4 instruments to be able to ensure that the questions are to be answered conscientiously by the respondents, the interview queries would be handled in such a way that it is based on professional pattern which means that the process of understanding in this phase of the process shall be based on the professional level of opinion of the staff members to be interviewed. Other Measures of Assessment to be Used As noted from the above notes and proposal basics for this research, it could be noted that the assessment on the medical institution’s use of SBAR system for hospital-to-hospital or doctor-to-doctor communication naturally helps in the process of assessing the kind of service that the institutions are giving their patients in time of their healthcare needs as well in handling some healthcare emergency situations. To assess the said procedural approach implicated through the SBAR system, understanding the standard quality measurement of the process shall be utilized for this research. The aspect of healthcare has always been a major concern of the present mainly because of its significance and its obvious values. Its primary concern is for the improvement of the present health condition of the people and the ethical concept of promoting life itself due to which, significant efforts and concerns are always given for the quality of healthcare services. With this idea, society constantly monitors and evaluates the quality of healthcare services being rendered by institutions regarding its promoting, implementation and improvement. Regarding healthcare service, hospital institution is the most common and the easiest way to access healthcare service from professionally trained personnels and individuals. Because of this concept, society always turns its needs whenever certain individuals seek professional health service to hospital institutions. Thus, closely monitoring the quality of the healthcare service and the administrative process of hospital institution is indeed beneficial for the society themselves. Through this actions, improvement measures and promotion of quality can be further develop for the advantage of the people thus certain essential tools and measures are developed for improving the quality of services provided by hospital institutions. Because of the said relevance and significance of the healthcare quality in the society, certain social groups and institutions have been created for the purpose of monitoring healthcare implementation and the administration of hospital institutions. Among the many created social institutions, the Agency for Healthcare Research and Quality (AHRQ) has been widely known for their support in the research field for the development and improvement of the healthcare services that are being rendered in the hospitals in the presents. In their aim of monitoring the healthcare quality of the said institutions, they have developed certain tools or modules designed to evaluate and monitor the effectivity and quality of the healthcare services in hospitals thus raising concerns and informations for improvements and further research on the part of the hospital’s administration. On the other hand, these modules can also be used by the public in evaluating the healthcare service they have received thus they themselves can also raised points and concepts regarding the quality and the systematic process of the hospital’s administration. In addition, this can also be used by the public in their process of creating their own informed decision and future consent. AHRQ’s primary concern is to raise the quality of healthcare services for the society mainly the ones being rendered by the hospital institutions through evaluating their present administrative system. Thus, they have developed different that measures various aspect of healthcare quality namely prevention quality indicators, inpatient quality indicators, patient safety indicators, and pediatric quality indicators. 1. Prevention Quality Indicators (PQIs) are mainly modules that can be used with hospital inpatient discharge data in identifying the quality of healthcare for “ambulatory care-sensitive conditions.” This module is primarily about the conditions for early treatment preventing further hospitalization of good outpatient and intervention procedures in preventing further complications of the patient’s health status. 2. Inpatient Quality Indicators (IQI) are mainly modules that provide perspective on hospital quality using hospital administrative data. This module actually reflects the quality of healthcare service being rendered by the institution inside their hospitals and the domain of their facilities. In addition, this module includes inpatient mortality for certain procedures and medical conditions; utilization of procedures for which there are questions of overuse, underuse, and misuse; and volume of procedures for which there is some evidence that a higher volume of procedures is associated with lower mortality. 3. Patient Safety Indicators (PSIs) primarily evaluates the health conditions and safety of the patient within the facilities of the hospital institution that includes reports for potential dangers, hospitalization complications, and adverse events following or before certain healthcare operations. This module helps system leaders and administrative division in identifying potential complications thus developing precautionary actions and developments. 4. Pediatric Quality Indicators are mainly modules for measuring the quality of service for pediatric healthcare basing from the hospital’s inpatient discharge data. This module specifically evaluates possible problems and complications brought about by the exposure of the pediatric patients to the healthcare system of the institution thus creating amenable preventive measures and actions and further improvement concepts on the institution’s pediatric system. Chapter 4: Conclusion Thus, the quality of healthcare service presently rendered by the hospital institution is significantly important for the promoting of the basic principles of healthcare itself. With this concept, indeed the institution’s healthcare administration should be constantly monitored to determined possible dangers and complications and thus, developing protective measures and safety programs for the development itself of the hospital’s service. Administering the said healthcare evaluating modules is also considerably meticulous and complicated at some point however, the benefits these modules can raise through providing important qualitative information is generally more relevant than previous aspect. Thus, evaluating the quality of the healthcare service being rendered by the hospital’s administration is indeed important as both a precautionary and preventive measures for possible complications and in developing further the present condition of the said institution for more efficient and effective healthcare service. Yes, with the SBAR communication system present in this particular field of understanding, it is expected that hospital-to-hospital process transfer would be of the best option in making the recovery of the patients come in a much faster and easier approach both for the healthcare staff and for the patients themselves.
