The professional nurse is responsible for initiating and updating the clinical care plan, the process of care provision so that it can be used for evaluation and guidance for client care. Nursing case management basically focuses on managing a group of clients and the role of nursing case managers are collaboration, identification, and evaluation of patient’s possible outcomes with respect to a certain time frame and the application of continuous quality improvement analysis (Masters, 2005).
