As a result, the healthcare system needs to deal with issues such as developing an advance care plan and coming up with ways to manage the comorbid conditions associated with chronic illnesses. At present, most inpatients with complications of chronic illness still lack access to proper care that emphasizes effective management of their condition. This leads to poor disease control and complicates matters for patients compared to an environment where there is emphasis on appropriate care or effective management.
