Therefore, systems to offer constant surveillance and to help patients in upholding blood pressure control over a period are critical to decreasing morbidity and mortality (Sudano et al., 2010). The enhanced use of evidence-based methods for managing hypertension and the growth of focused care management curriculums that encourage collaborative care have the perspective to increase enormously control rates. This method has been seen to achieve the best control measures hence reducing the occurrence of CAD. Furthermore, lifestyle counselling, home monitoring, and proper medication management, especially in the context of a structured, evidence-based method for long-term governing, constitute a robust tool for effective management of hypertension and enhanced health results for the hypertensive patients (Health, 2015).
