Mr. P’s Case Study Solution Solution The situation facing Mr. P and his wife is a complex issue due to the need to maintain diet restriction, managing his polypharmacy and the task of caregiver, which becomes difficult for his wife due to financial constraints and Mr. P’s depreciating health. The best strategy to approach Mr. P’s care is to advocate nutritional therapy as prescribed by the physician. This will keep his body strong and able to fight against other diseases that may attack the patient.

The caregiver’s knowledge on the quantity of fluids and other dietary check will limit complications that lead to the patient hospitalization and increment in costs. The primary objective being treatment of the patient is important for the patient and his wife to comply with the prescribed medication with the help of an outpatient’s nurse supervision. The medications include ACE inhibitors and beta-blockers, which will improve the health of the patient by stabilizing the patient for more therapeutic measures (Escott-Stump, 2008).

The patient should avoid alcohol and nicotine due to the difficulties in breathing and his lungs condition.

Engaging in physical activity enhances relaxation and improves the physical health of the patient thus reducing hospitalization and health care cost by more than 40%. Recording of the patient’s weight on his diary will help monitor the responses of the approach plan. The outpatient nurse’s supervision will help ensure improvement of the home care (Meerabeau, & Wright, 2011). Assessing the patient’s and family health literacy will help the counselor to know the best strategy to educate the patient and family.

The counselor will educate the patient and the caregiver on the importance of changing their lifestyles; manage fluid balance and the whereabouts of the implantable cardioverter defibrillators.
