Survey and/or Interview Questions are designed for trainees, their immediate supervisors, their subordinates and others who frequently observe their behaviour. A control group should be set up. A survey should be conducted before and after the course. The after course survey can be conducted in intervals of three, six and nine months after the course.

Level 2 Questionnaire
Goal: To discover how much the trainees knew before, during and after the course/programme.
Questions are asked on: course subject matter; its application to workplace and the individual.
The Questionnaire
1. How much do you think you knew about the subject matter of this course BEFORE you took the course {5 =Very much; 1 = nothing; 2 = little; 3 = average; 4 =much}
2. A short Quiz {Multiple choice; True-false} is designed by the instructor and administered after the answer to question 1 of this Level 2. This serves as pre-course knowledge base.
3. As each module of the course is completed, a Quiz is administered to measure the level of learning during the course.
4. After the course, {say one week} another quiz is administered to measure the learning that actually took place: how much retention of the course information.
5. Has this course improved your knowledge of the subject matter
{ 5 = Very much; 1 =not at all; 2 =little; 3 = average; 4 = much}
6. Where will the acquired knowledge be most useful
{5 = organization; 4 = Personal; 3 =4+5 ; 2 = Not sure; 1 =no where}

Summary

In designing these training evaluation questionnaires, we have followed the Kirkpatrick four-level model and the Phillips Return On Investment (ROI) framework. Thus we produce questionnaires that measure, before and after training, the main issues raised by these two evaluation experts as summarized in table 1 below.
