2007 Performance Evaluation l: Fulfilled the Duties of an Administrative Manager Executive Rating CEO Rating: Results: As a key member of theExecutive Office my performance has been excellent. My work as an Administrative Manager has been outstanding. Throughout the year, I have been able to demonstrate my initiatives, my leadership, and tenacity in performing tasks. I have managed my assignments to purposely fulfill my tasks on an ongoing basis and on long term projects. Goal: Improved my role as liaison to the CEO Executive Rating: 1 CEO Rating: Results: As liaison to the CEO, I was able to interact with senior management, external colleagues, and the board members in a dependable and responsible way. Although I am not listed as a key contact person in the Board Directory, I am usually sought as a person with whom to exchange information. I am often requested by board members and external clients to prioritize requests: concerning the use of VNS and other VNA´s contacts; benefactor inquiries concerning donations; and, seeking the use of VNS´ facilities for external events. I effectively performed positively as a liaison and used an objective approach. Goal: Predicted Executive Office’s 2008 Operating Budget Executive Rating: 1 CEO Rating: Results: I significantly contributed with the Finance interface and the PeopleSoft Budgetary System. I made a difference by: Formulating financial forecasts and variance reporting, therefore successfully determining the 2008 budget for the Executive Office cost center 9500 and the Board Administration cost center 9616; Helping the Board Liaison in projecting a budget line for the 2008 external events; and, Serving as an advisor to the administrative assistant in categorizing expenses, inventory control, and compliance with the rules and regulations for the Procurement Division. Goal: Supervised the administration assistant Executive Rating: 2 CEO Rating: Results: I successfully supervised, guided, taught, and motivated the administrative assistant. I successfully oriented the new administrative assistant to the corporate training manual. With my input he was able to satisfactorily carry out his duties. Goal: Administered and organized internal and external appointments for the CEO Executive Rating: 1 CEO Rating: Results: I successfully managed my workload and continued to successfully attend to the scheduled internal meetings. I proactively, under the CEO and the help of project managers coordinated VNS meetings (SMG, ISSC, MCS), cross-agency initiatives (Infusion, Workmen’s Compensation, PPS), business growth meetings (Congregate Care, LTHHC, GFS), staff meetings VNS Roundtable, and individual staff meetings. I also scheduled meetings for the CEO´s direct report and with external colleagues. I also: Monitored meeting schedules and made appropriate changes. Gathered briefing materials and inquired about formats for off-site meetings. Goal: Immediate Actions Toward the Patient’s Complaints Executive Rating: 1 CEO Rating: Results: As an intermediary to the Executive Office, I handled patients´ complaints and other pertinent matters. I documented the complaints thoroughly and sent them to the personnel in charge of the resolution. I met with the CEO and senior management on critical matters in confidentiality and processed the information to the personnel that would look into the complaints. Goal: Research: PLEASE ADD A GOAL FOR RESEARCH AND A DESCRIPTION Part B: Major Accomplishments - Describe any additional accomplishments that you would like to have considered as part of your performance appraisal. I have multi-functions within the Executive Office which I display with excitement. On behalf of the Executive Office I worked with various companies: AFB, Barrier Therapeutics, CCLC, GNYHA, Lifetime, and Pace. I helped in preparing disclosure statements and conflict of interest forms, calendar schedules, securing signatures, and the distribution of contracts and grants. Quite often, I had to change directions but I managed to accomplish the duties in a professional matter and maintained communication with the internal and external colleagues when soliciting information from them since I have gained their respect and trust. SECTION II: COMPETENCIES Performance Rating Focus Area Strategic Management The CEO is given support by setting and achieving the organizational strategies as follows: Assuming leadership in management changes. Communicating to the staff the reasons for strategies and the importance of achieving them. Implementing the projects´ plans within their budgetary constraints and deadlines and promptly addressing difficulties. Promotes measurable improvements in all areas of responsibility; identify areas and processes where there in space for improvement to obtain more efficiency and/or effectiveness. Monitors areas of responsibility to identify and address areas of risk. Takes proactive action. Informs the CEO of emergent organizational issues. Proactively keeps updated of new trends in healthcare, public policy, market place, and community which may affect VNSNY. Elaborates responses to enable VNS to adapt and to maintain its strong position in the future. Analyses and provides data for decision-making. Analyses the cost-benefit realm to make sure that its value comes from resource allocations. Employee Rating: The CEO trusted my abilities to solve matters within the Executive Office. I organized the tasks and time to give priority to the activities to take place so that the CEO could delve in other issues. CEO Rating: Results-Orientation Action, results, and achievement were established for target performance. Employee Rating: I worked with diligence while focusing on deadlines. I was able to use the resources available and when in need I sought the help of others. I work well with the staff and external clients and I am always thinking one step ahead. . CEO Rating: Communication Communicates in verbal and written form the ideas and points of view in a clear and concise manner. Maintains communication with agency colleagues within the organization and with other stakeholders. Prepares the evidence for the findings and concludes in a convincing fashion. Employee Rating: I worked out differences between vendors, VNS staff, and external clients in an efficient manner. These are as follows: I assisted in the reconciliation of contractual disputes with vendors. I gave input to the Procurement Division concerning the travel policy for the CEO. I worked with the trade organization’s (HANYS, NYHPA, and HCANYS) liaisons to schedule meetings for the NyeC Board Members. These meetings were to coordinate the schedules of 15 participants from New York, Albany, Syracuse, and Glen Falls. CEO Rating: Initiative and Innovation Thinks creatively and is inventive, proactive, and a problem solver. Employee Rating: I constantly seek new venues to improve productivity. Due to the CEO´s appointments to new committees I have had to work with the schedules. I continue to communicate with the liaisons of the other organizations to make myself known to them. I was able to schedule the CEO´s independently of the demands made by external colleagues. I was able to succeed in negotiating solutions to problems that surfaced at the last moment, to obtain common goals due to my creativeness and innovative approaches. CEO Rating: Organizational Development Organizes a strong and diverse team for the completion of their responsibilities and accountability Renders feedback and gives positive recognition, and keeps track of employees not meeting expectations. Incorporates competencies and skills of staff and allows them the opportunity of career growth Fosters and maintains the talented staff for future endeavors Promotes different points of view and acknowledges positive and negative information Employee Rating: Maintained a close association with the Human Resources Department to identify the best candidate for the administrative assistant position in the Executive Office. I recommended an internal co-worker to the position of administrative assistant in the Executive Office with the belief that this person would handle the diverse responsibilities according to the job portfolio. CEO Rating: Interpersonal Skills Shows the ability to work efficiently with other functional areas and is a team member. Employee Rating: I continue to maintain a good relationship with all of the CEO´s vice-presidents and outside colleagues. I have been successful in being an integral member of the Executive Office staff and partaking in the horizontal and vertical flow of information to maintain the stability of the office. CEO Rating: Customer Service Maintains proactive relationships with internal and external customers. Identifies the needs of internal and external customers for customer satisfaction. Gathers customer feedback and takes action to improve the service. I always presented myself professionally as a liaison on behalf of the CEO and the echelons of management. These are as follows: I have followed up on all of the tasks that were assigned to me that led to customer and client satisfaction. I successfully coordinated the facilities for the external events. I successfully contributed to the budget and financial matters through positive input. I made adequate and positive predictions concerning the finances and the budget. I made positive contributions while working with liaisons. As an advisor to the administrative assistant, I successfully categorized expenses, maintained inventory control, and sought measures for compliance to the rules and regulations of the Procurement Division. I successfully oriented and trained the new administrative assistant. I became involved in the recruiting process for the administrative position by making recommendations of persons that were qualified and matched the job profile. I prioritized the CEO´s meetings to make sure that the top of the list matters were taken care of and were resolved as soon as possible. I have contributed to making the healthcare organization a leading one of its kind by rendering consistent quality, including patient-centeredness, being more efficient (effective utilization of existing resources and reductions in expenditures), and being a thought leader and innovator in reshaping the healthcare system. I handled the patient’s complaints successfully and followed up with documentation and sent it to the relevant agency that needed to address the matter with confidentiality and resolve the matter. Due to my multi-tasking, I was able to handle many assignments and obtained positive results. I implemented projects that had obtainable and measurable results. I monitored and identified risks that would help to prevent problems. I was able to address and communicate to the staff the rationale for some decisions that were made and how their role within the organization would help reach these decisions. By keeping abreast in the new trends in healthcare issues, public policy, the marketplace, and community; I was able to give input concerning these matters. I conducted cost-benefit analyses to make sure that the value derived from resource allocations. I worked creatively and made suggestions to solve problems. I provided feedback to employees to help them meet what was expected of them. I helped the staff develop their competencies and skills to give them a better opportunity for career growth. I developed plans to retain the talented staff. I opened up the communication to receive positive and negative feedback. I helped maintain constructive relationships with the internal and external customers. I obtained customer feedback to improve the service.
