"Community Health Problem and Improvement Plan" is an outstanding example of a paper on the health system. A community health improvement plan is an organized effort that tries to address a health problem in a community; it is established from the outcome results obtained from community health assessment activities. This type of plan is created collectively, and it outlines a community health vision. Besides, this plan guides all the stakeholders, which include community health boards and partners on how they should work in improving the health issues under their prerogative.

According to Laymon et al. (2015), it is vital to formulating policies and actions that help in health promotions, as all the stakeholders can use them in setting priorities, organizing, and steering resources. This paper will try to discuss mental health care for minorities (blacks and Hispanics) as one of the health problems in our community health improvement plan. Again, the paper will give the structure, process, and outcome standard that will be used to evaluate the program addressing this mental problem.

The minority group experiences mental disorders, just like any other person, which may include depression, bipolar disorder, schizophrenia, and anxiety. Still, in most cases, they are far less likely to receive treatment for the same. This may be due to the following reasons; They may lack access to proper medical care as, in most cases, the percentage of medical access is directly proportional to one's level of income. Blacks and Hispanics have a higher poverty rate and more likely to lack medical insurance.

 Language barriers that may hinder them from receiving proper mental healthcare, when being diagnosed and treated, should accurately express the symptoms to the physicians to understand the steps that will be needed for the treatment. The language barrier may discourage the sick person from pursuing treatment. Cultural beliefs, expectations, and behavior have significantly affected mental illness treatment for most blacks and Hispanics. Most of them see mental health disorders as shameful sickness, and due to such stigma associated with the sickness, most of them shy from getting treatment (Laymon et al.,2015).The Donabedian model will be used to measure the structure, process, and outcome of this program, and it will be used to study mental health services and assess the program's quality, it has a three-component approach; the structure, the process, and the outcome.

Donabedian believed that the structure affected the process, which in turn affected the outcome.According to Medved et al. (2001), the outcome depicts the result and the impact it had on the people with a mental health condition and if it has achieved its intended results (s). For instance, improved mental patients experience. The process depicts the way the system work to achieve the intended outcome, for example, the time the patient waits to get a review from a physician and finally the structure which depicts the attributes of the service provider such as the staff ratio to patients’ ratio, the operating hours and any other input that the service requires (Pennel et al., 2016).ConclusionCollective responsibility is vital to improve the mental health care program, which entails coordination across different service providers.

Factors that hinder service delivery to the minority group need to be addressed to ensure everybody has access to proper mental health services. Practical standards should be used in evaluating the quality of services offered to the patients.
