"Disseminating Evidence" is a wonderful example of a paper on the health system. The theory of dissemination is constant with Merriam-Webster's first definition of dissemination: "to spread abroad as though sowing seed". In the public health community, disseminating evidence refers to alerting the prospective adopters through medical journals, guidelines, or programs about new information which could be adopted quickly in the clinical settings. In other words, dissemination of evidence refers to a string of planned activities proposed to encourage and enable acceptance and implementation of proven protocols. For this author, data on evidence-based guidelines for Ventilator-Associated Pneumonia (VAP) prevention should reach the National Nosocomial Infections Surveillance system so as to convert it into a success. The author starts at the bottom with the local YMCA since reaching the NNIS may be tough and starting at the bottom could be an easy way to reach the top.

Plans for Disseminating Evidence

       The dissemination of evidence-based practices through sustainable distribution channels is vital for endorsing and conveying guidelines-based programs to prospective adopters and improving access to easily implementable programs. The prospective adopters of the author’s guidelines are the hospital ICU nurses and all the other ICU staff who directly take care of the critical patients. The author of the evidence guidelines should approach the local YMCA and their national organization for the diffusion of their evidence-based guidelines. These YMCAs should further collaborate with the NNIS system to create an online site or journal to augment the use of research data and evidence and assist the selection of evidence-based VAP prevention programs. Many hospitals are members of the NNIS system and therefore, by approaching this system, the guidelines could be implemented widely (Maibach, Van Duyn & Bloodgood, 2006). The author should remember that the proposed guidelines should clearly define the problems, the review of the meta-analysis done in various hospitals, and the proposed solutions to get approval for the article from the YMCA. Moreover, it should provide the process to evaluate implementation and conformity with clinical practices recommended in the guidelines, compares rates of VAP before and after implementation of the guideline suggested, and scrutinizes the patterns and associates of changes in rates of VAP. The dissemination plan should conform to the points, include statistics, the appropriate plan, and the process of how this plan could reduce the VAP rate considerably (Larson, Quiros, &Lin, 2007). However, in case the local YMCA is not convinced, the author should attempt at approaching the various government health organizations, health care professional societies, health care quality improvement organizations, or voluntary health organizations. Indeed, getting the guidelines published in an online nursing journal will not prove only fruitful for the author but help him gain acknowledgment and recognition simultaneously.

                                                           Conclusion

            The local YMCA is the best place to start dissemination of guidelines since starting from the bottom might prove better to improve the environment and social ecology of our communities in evidence-based ways. The YMCA and NNIS will definitely boost the dissemination of evidence-based disease prevention guidelines, and the specific programs and services that are steady with these guidelines have substantial prospective to promote healthy living, directly and indirectly. Therefore, initiating from the lower level will hopefully reach the policymakers and the plan can begin. In case it does not work, the author should try going straight to the highest level or get the article published in a journal.
