"Care Coordination" is a wonderful example of a paper on care. Care coordination is among the roles of a nurse leader. It involves the organization of patient care activities between different healthcare team members, including nurses, physicians, pharmacists, and social workers, among others. The nursing leader ensures that the involved healthcare members develop positive relationships so that they can work collaboratively to improve patient care outcomes. In this paper, I will discuss how I would play the care coordination role to ensure that the interdisciplinary healthcare team members work collaboratively to achieve their patient care goals. The patient in my scenario is Sharon Haskins. She is a 56-year-old patient admitted to the hospital with diagnoses of uncontrolled diabetes mellitus and renal insufficiency. In this case, I will apply my coordination skills to ensure that she undergoes effective collaborative care.

First of all, as a nurse, I need to delegate some of my duties to focus on coordinating the whole healthcare team as they offer care to the case patient. The duties to consider for delegation include direct patient care, updating the patient history, and managing healthcare inventories. Having delegated some of the duties, I will develop a strategy to enhance communication and collaboration among the staff members in different disciplines. Besides, I will introduce other workers assigned to the unit so that they understand what they need to do and learn to collaborate with other healthcare workers. According to the study done by Karam, Brault, Van Durme, & Macq (2018), interprofessional education allows each healthcare personnel to understand and appreciate other professionals in the healthcare field. Therefore, I will ensure that all healthcare team members understand the importance of interprofessional collaboration before initiating the care of the case patient.

The team of Registered nurses will offer general nursing care to the patient., which includes administration of the prescribed medications, e.g., metformin glipizide, lisinopril, and morphine sulfate. Other nursing care practices for the case patient include blood sugar monitoring, monitoring the patient for complications, and teaching the patient about lifestyle modification such as taking a proper diet and exercising regularly. To perform these activities, nurses need to collaborate with other healthcare workers. Firstly, effective collaboration and communication with physicians allow mutual consultation, leading to avoidance of medical errors that result due to inappropriate medical prescription or administration (Johnson & Carragher, 2018). Secondly, I will as well coordinate the collaboration between nurses and the registered dietician. Diabetic patients required an appropriate ADA diet to avoid an unobserved rise in blood sugar levels. Therefore, collaborating with the dietician will nurses to achieve this goal.
The case patient also needs to start insulin treatment, which is mostly administered by nurses. However, nurses need to collaborate with a physician specialist, i.e., an endocrinologist who has more knowledge regarding the management of diabetes mellitus. In this case, the endocrinologist will prescribe insulin therapy so that nurses can administer it to the patient to stabilize blood sugar levels (Johnson & Carragher, 2018). Lastly, I will coordinate the collaboration between nurses and pharmacists to promote a continuous supply of prescribed medications to improve patient care outcomes.
