Patient-Centered Care Report

Introduction

Patient-centered care is care delivered to patient by listening to the patient and informing them about the care. Patient-centered care gives patients the opportunity to learn about healthcare and this helps them to improve their experience in healthcare. Patient centered is an important strategy to use because it helps to reduce healthcare cost and improve patient outcome. Patient centered care focus of delivering effective care to improve the outcome. The patient centered care report will report the strategies that were used to improve care delivery.

Evaluation of the outcome of a population health improvement initiative

Exercise made the outcome achievable. Patients who participated in the exercise started showing improvement in the first four week. The population health improvement focused on the elderly patient with TBI and due to this exercise was considered to be important because it will help them improve. He exercises that were introduced to the patients became helpful to the patients. Patients who continued with the exercised showed great improvement where they scored muscle control 15%, mood 22% and short to medium memory 61%. The improvement result were better to patients who continued with exercise after three months.

Medication and therapy; to get the right information about the patient in regard to the condition, all patients underwent test. Physiotherapist conducted the test and it as discovered that some patient had not been under treatment and they were immediately introduced to medicines while the patients who made been under medication were introduced to therapies. Testing he patients was considered to be important because this was to determine the leaver of need.

Meditation was another initiative that was introduced to the patient. The meditation was to be done at home for about 10 to 15 minutes a day and patient who participated showed much improvement by 70%. During the process most patient were lost on the way. Some patient did no complete the process and this can be associated with poor communication. The outcome was not fully achieved due to some patient dropping on the way.

The aim of the community improvement initiative was to help all patients with TBI but it was discovered that some patient did not complete the process. Communication as not well conducted. Patients at the begging were not told what is expected of them and how the initiative can improve their health. Lack of information can be the cause of the patient not completing the process.

Proposal of a strategy that can improve the outcome of population health improvement initiative

Poor communication was one of the factors that was noted to reduce the patient outcome. To improve this it is important to have situational briefing (Woulfe et al., 2017). It is important to brief participants in every session they will be taking. At the begging of the session it is important to tell the participants the importance of the session and the anticipated result. When patients are aware of what they will go through and the expected result the participants will work hard to achieve the outcome. Communication is important because it will motivate the participants to complete the sessions and this will improve the outcome. From the scenario we see a high number of patients at the begging started the treatment but a small number could complete the treatment this shows that most patients did not understand the importance of the treatment.

Second, the environment might have affected the outcome. The exercises that were introduced to the patient did not consider age. It was good to group patient according to their age this is because different exercises are effective to different groups. For example it is hard for elderly patients to exercise their memories using Sudoku thus grouping patient could be of great help and try to find exercises that favor age.

Approaches to an Individualized Personal Care Plan

Following the outcome and lessons learned it is important to have personal care to TBI patient. Different patients have different ability and due to this grouping patients might affect the patient outcome (Rader et al., 2016). It is important for follow patient cases individually because this will help the care provider to monitor and helps the patients during the process. Different patients have different needed thus when conducting care different it becomes easy to make a follow up of the patient and deal with the patients needed that will help to improve the patient outcome.

Second, environmental and economy are major causes of lack of patient outcome (Rader et al., 2016). Following the case of the participants in the PHIPI some patients did not complete the treatment this is because of grouping as it was suspected thus working with an individual, things alike exercise could not be a problem because caregivers will choice the appropriate exercise considering the age. lastly different patient a have different culture and due to this working as a group might be hard because they might be forced to do things that their culture does not allow, therefore it is important to work with individuals as this will improve communication. When a patient and a caregiver communicate directly it will be easy for them to agree on what is effective to the patient as what might work in one patient might not work in the other patient.

Value and relevance of evidence used in the patient care plan

Personal patient care is an important approach this is because it helps patient acquire knowledge on how care is conducted thus improving their experience (Goldsmith et al., 2017). Several studies that were conducted to determine the effectiveness of PPC found it effective. From the scenario most patients did not complete the treatment because it was conducted in general. Different patients have different need thus was conducting personal patient care it becomes easy to note the needs of each patient making it easy to address individual needs unlike when conducting community care.

According to Goldsmith et al. (2017), personal care is also vital because considering the factors that affect the patient outcome; patient uniqueness and patient family are the main cause; therefore, addressing personal issue can be effective because care is conducted considering the patient and the patient family. Focusing on an individual health issue it will be easy to address the issue as this will help to look at the causes and also the best approach that can help to resolve the issue.

The desired outcome is to have a health population. To achieve the outcome all patient with TBI will be educated in the importance of undergoing treatment (Woulfe et al., 2017). Treatment is important because it will help patient stay healthy and this will result to a health population. The aim of the healthcare industry is to have a health nation and due to this it is important to involve all patients with the TBI condition. Patients to accept to engage in treatment, creating awareness is essential. Some patients do not go for treatment because they are not aware of the treatment or they do not know the importance of treatment.

Educating patients on the importance of treatment is an important measure because it will help the patient to engage to the treatment willing and this will make them complete the treatment as required (Reder et al., 2016). It was noted that most patient could not complete the treatment and this was associated with lack of health care information thus educating the patient will, make them have desire to complete the treatments since they will know what they will achieve at the end of the treatment. Creating awareness is important because it makes patient understand what they can do to stay healthy also creating awareness improves the experience of patients and this way patient can take care of themselves with minimal helps and this will reduce care costs. Patient experience is important when using PPC approach. Patient who have experience have been noted to heal faster than patients with no experience.

Population health improvement care is an important approach because it helps to improve the care of the community. Different communities have different issues that require different approaches. Patient centered care is an important approach in healthcare because it helps to improve the experience of patients and this helps to reduce care costs. The report has found out the outcome was achieve due to the exercise, treatments and the meditations that was applied. On the other hand communication is an important aspect when it comes to health care. Healthcare providers when they communicate to the patients it becomes easy for them to come up with a solution that will favor the patients’ needs thus the patient outcome.
